What does a monsplasty (pubic lift) do?

A monsplasty, or pubic lift, lifts and flattens the mons — the pad of tissue over the pubic bone — by removing skin that has descended after pregnancy or major weight loss and thinning the fatty layer beneath. Dr. Wai-Yee Li performs it in Arcadia and Pasadena, Los Angeles, most often at the same time as a tummy tuck.

A flat, firm abdomen can still be let down by the area just below it. The mons — the soft pad of tissue over the pubic bone — can sag, thicken or hang in the same way the abdomen does after pregnancy, major weight loss or simply with time. A monsplasty, sometimes called a pubic lift, lifts and flattens it. Dr. Li performs monsplasty in Arcadia and Pasadena, Los Angeles.

What a monsplasty addresses

  • Excess skin over the pubic bone — skin left loose after pregnancy or major weight loss, including weight lost with GLP-1 medications.
  • A mons that has descended — tissue that no longer sits where it once did, so the area reads heavy or full under clothing and swimwear.
  • An area a tummy tuck alone does not reach — a beautifully flat abdomen above an untreated mons can be let down by the contrast; treating both keeps the contour continuous.

Dr. Li's approach

A monsplasty is most often done at the same time as a tummy tuck, through the same low incision, so there is no separate scar to account for. Dr. Li removes the excess skin, thins the fatty layer where it is contributing to the fullness, and lifts what remains so it sits naturally against the abdomen above it.

As in her abdominoplasty work, the incision is planned to sit low, where clothing and swimwear cover it, and closed in layers so it settles into a fine line.

Often part of a bigger picture

A monsplasty is most commonly combined with a tummy tuck. After major weight loss it is often considered alongside an arm lift, or with breast surgery — a breast lift or reduction. What can safely be combined is decided at consultation, always with recovery in mind.

Frequently asked questions

Most often, yes. Doing them together means one incision and one recovery, and it keeps the contour continuous from the abdomen down. Whether they are combined is decided at consultation, always with recovery in mind.

When it is done with a tummy tuck, the same low incision is used, so there is no additional scar. Dr. Li plans it to sit low, where clothing and swimwear cover it, and closes it in layers.

Only if the problem is fullness rather than loose or descended skin. Liposuction removes fat; it cannot lift skin that has stretched. Where the skin itself has descended, it needs to be removed and lifted — that is what a monsplasty does.
